We understand that you may feel anxious about your first visit to our center. You can be certain that we will do everything we can to make you and your family comfortable. Please let us know if there’s anything we can do to make you feel more at ease.
Schedule an Appointment
To make an appointment, please call us at 573-875-7081 Monday through Friday, between 8:30 a.m. and 5:00 p.m. Our practice is located at 1705 East Broadway, Suite 100, Broadway Medical Plaza #4 in Columbia.
Forms
During your first visit, you will be asked to complete several forms. To save time for you, we encourage you to print and complete these forms at your convenience and bring them with you to your appointment.

Insurance
During your first visit, your financial counselor will meet with you to review your insurance information and discuss what your insurance will cover. This allows us to file claims for all office visits and in-office procedures on your behalf and send out monthly statements to keep you informed. For information about your specific plan, contact a financial counselor at 573-874-7800. If you do not have insurance, the financial counselor will work with you to set up a payment schedule and provide you with information about agencies that offer assistance.
What to Expect
We encourage you to bring another person with you to your first appointment. When you arrive at any of our locations, you will find convenient, easy access into our center, even if you require a wheelchair. During your first visit, you should plan to spend an hour, and the appointment may include:
- meeting and evaluation with your physician
- tests needed for an accurate diagnosis that will guide your treatment
- meeting with your nurse and other members of your medical team
- meeting with your financial counselor to discuss your insurance and billing questions
